The cornerstone of our supportive approach is collaborating directly with residents and their families/care teams to design customized action plans nurturing personal growth during stays with us. While some individuals join our independent living training straight from hospitals or juvenile facilities, others may experience relapses requiring return admissions to stabilize. Wherever one starts their journey, through initial comprehensive assessments, we identify current ability levels across skill domains like household, vocational, emotional and community functioning. Residents then self-determine priority goals like learning laundry for more domestic freedom or better recognizing anxiety triggers before outbursts. With holistic insights gathered from assessments, we mutually establish incremental support steps towards target milestones. Then via regular progress conversations, we recalibrate plans as needed incorporating case managers’ and clinicians’ perspectives to propel momentum for individuals alternately prepared to attempt new challenges or needing additional reinforcement first. During quarterly progress meetings, we celebrate small wins like social hour attendance or neighborly interactions while adjusting accommodations, counseling approaches and life skills lessons accordingly. As confidence builds over the months or years, transitions are gradually introduced through group home visits offering rigorous assessments for those aspiring. When individuals do outgrow facilities, having witnessed firsthand their courage battling adversity, we feel privileged to have empowered them recognizing their own inner resilience and strength to continue thriving independently. Though goodbyes are bittersweet, we remind graduates – LS Genesis will always be family so visit anytime.
